Ultimate Wellness Massage LLC does not provide billing services. We will simply provide receipts and/or copies of records for clients to submit for reimbursement. A physician referral demonstrating medical necessity may be  required for insurance reimbursement/health savings account reimbursement regardless of who submits bills. Ultimate Wellness Massage LLC will not be responsible for claims that are denied and can not give any advice.

Billing or medical charting for legal or insurance purposes needs to be requested upfront. Depending on the client, if it was not discussed prior to service that it was for medical and/or legal  purposes, Ultimate Wellness Massage LLC may or may not have the information needed.

When seeking reimbursement for medical and/or legal purposes, Ultimate Wellness Massage LLC can not apply any discounts, member/package rates, gift cards or third party promotions.

Refund policy

Ultimate Wellness Massage LLC has a no refund policy.

If you have received or purchased a gift card or membership/ package and are no longer able to receive services please let us know who the remaining services will be gifted to via email Ultimatewellnessmassage@gmail.com

If a refund is unavoidable, used services will be charged at the full service price and there is a $35 administrative fee.

There will be a 3% Non Cash Adjustment (NCA) on the balance if the purchase was made within 12 months.

For payments made with credit/debit over 12 months, cash or check the client will incur charges for cashier check and postage.
